About Us

Moffett Physical Therapy provides high quality care in a comfortable friendly environment. Our slogan is “Building a Pathway to Better Health and Fitness.” Fitness is a stepping stone for good health and good health sometimes requires intervention from a skilled professional such as a physical therapist. Our physical therapists are experts in exercise prescription and joint and muscle dysfunction. We are physical therapist owned and proud to have three convenient locations just off Route 90, in Huntley and Hampshire and now in LITH at the corner of Lakewood and Ackman.

What is a physical therapist?

Physical therapists are highly educated professionals, who are now graduating from Universities with a Doctorate of Physical Therapy. Physical therapists are experts in rehabilitation of muscles and joints, injury prevention and prescribing exercises that are appropriate to the individual. Our physical therapists are orthopedic trained. We treat all ages of people who have soft tissue and joint dysfunction.

What to expect...

Our physical therapists will do a thorough examination of your body and how it relates to your pain and symptoms. We will then determine a treatment plan unique to your dysfunction. This treatment program will help you obtain the best possible range of motion, strength and most importantly… function. During your entire treatment process we will communicate effectively with your physician in order to address your pain and symptoms with care and respect.

About the Owner, Cheryl A. Moffett, M.P.T.

Cheryl is a 1993 graduate of Northwestern University Medical School, School of Physical Therapy, Chicago, IL, with a Masters degree in Physical Therapy. Cheryl completed her undergraduate at University of Washington, Seattle, WA, in 1991 with a Bachelor of Science degree in Zoology. Cheryl has had extensive training in Manual Therapy of spine and extremity. Her course work also includes craniosacral therapy, visceral manipulation, certification in pregnancy and exercise and athletic training at a collegiate level. She has directed outpatient orthopedic physical therapy centers, provided acute care, and participated in specialized committees. She is a member of the American Physical Therapy Association and very active in the Huntley Area Chamber of Commerce. Cheryl is friendly and dedicated to her patients, always providing professional and quality care.


Deborah A. Mitchell

Education:

  • Received her DPT from Temple University in Philadelphia, PA in 2006
  • Masters in Physical Therapy from Bradley University in Peoria, IL 2002
  • Bachelor of Science degree in Health Science from Bradley University in 2000.
  • Debbie has taken additional training in:
    • Vestibular Rehabilitation
    • Parkinson’s Rehabilitation
    • Balance and Fall Prevention
    • And is a Certified Golf Fitness Specialist

Debbie joined Moffett Physical Therapy in the early summer of 2006. Debbie says, "The most rewarding part of my professional career is the ability to help individuals return to his or her normal activities." Outside of work Debbie enjoys reading, gardening, walking her dog, golf, and spending time with friends and family.

Elsie Jakubowski

Education:

  • A 1997 Graduate of the Universidad Industrial de Santander in Colombia, South America with a Bachelor Degree in Physical Therapy

Elsie came to Moffett Physical Therapy in the January of 2006. Elsie speaks English and Spanish fluently. Outside of work Elsie enjoys watching movies, outdoor activities, spending time with her family (especially playing I spy with her son). Elsie says, "The most rewarding part of my professional career is being able to help people improve their quality of life."

Jenny Taylor

Education:

  • A 1980 graduate of Northwestern University, Chicago, IL with a Bachelor of Science degree in Physical Therapy.
  • Certified as a clinical specialist in Orthopaedic Physical Therapy by the American Board of Physical Therapy Specialties (ABPTS) in 2006.
  • Extensive experience treating spinal disorders using a manual therapy based approach.

Her other interset areas include treatment of temporomandibular, cervicogenic headache and shoulder disorders. She has had past experience in pain management, occupational health, industrial consultation and general rehabilitation. She is an active member of the American Physical Therapy Association (APTA) and the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT). She is dedicated to continual improvement of her clinical skills in order to provide quality care for her patients. Jenny says, "I am thankful that I have always loved my work as a PT. Not everyone can say that about their chosen profession." Jenny and her family are the quintessential RV campers whose personal goal is to visit every National Park in the US.

 

 

Heidi Risteca

Education:

  • A 1983 graduate of Oakton Community College in Des Plaines, IL with an Associates Degree of Applied Science in Physical Therapy
  • Heidi has taken additional training in:
    • Rehabilitation of Sports Injuries
    • Rehabilitation of Orthopedic Injuries
    • Course featuring material from: McKenzie, Mulligan, and Scapular Rehabilitation
    • Certified Kinesio Taping Practioner

In February of 2006 Heidi came to Moffett Physical Therapy with over 20 years of outpatient orthopedic experience and works in Both the Hampshire and Huntley clinics. Heidi says, "I have treated many patients over my professional career and I have learned in order to communicate effectively you must first be a good listener." Heidi lives in Hampshire and enjoys dancing, theater, music, and spending time with friends and family.

Laura Kent

Education:

  • A 1991 graduate of Oakton Community College in Des Plaines, IL with an Associates Degree of Applied Science in Physical Therapy
  • Laura has taken additional training in:
    • Stability and Fall Prevention for Seniors
    • Upright Control for CVA patients

Laura came to Moffett Physical Therapy in November of 2005 with over 15 years of orthopedic experience. She feels the most rewarding part of her job is helping her patients achieve the goals they have set for themselves. When Laura is away from work she enjoys spending time with her family. “It’s nice to work in the town I live in. Many of my patients have been neighbors or teachers from my children’s school. Our clinics definitely have a small town feel and provide a great environment for the patients and our staff.”

Carrie Lyne Mathiesen

Education:

  • A 1998 High Honors Graduate of Elgin Community College in Elgin, IL with an Associate Degree of Applied Science in Physical Therapy.
  • Bachelor of Science in Biology, Bachelor of Arts in Applied Psychology at the Universty of Illinois at Chicago, May 1994.
  • Carrie Lyne also has taken additional training in:
    • Geriatric Orthopedics
    • Orthopedic Rehab of the knee and shoulder
    • Manual Myofascial Release Techniques
    • Certified Weight Trainer

In April of 2006 Carrie Lyne came to Moffett Physical Therapy with 8 years of outpatient orthopedic experience and works primarily at the Huntley clinic. "The best part of my profession is being able to not only help poeple feel better, but educate them on how to help themselves", says Carrie Lyne. She resides in Hampshire and enjoys sports, landscaping, decorating her home, cruising in her 1967 Ford Mustang and keeping active with her three children.

Gayle Monaco

 

Education:

  • A 1994 graduate and Presidential Scholar at Oakton Community College in Des Plaines, IL with an Associates Degree in Applied Sciences in Physical Therapy.
  • Gayle has taken additional training in:
    • Post-surgical rehabilitation for knee, shoulder and lower extremities.
    • Rotator Cuff and impingement disorder.
    • Functional Testing for program design of patient care.
  • Gayle has also been certified as a Personal Trainer through NASM since 1999.

Gayle's interest in PT began with an ACL injury while playing in her church softball league. Her subsequent physical therapy planted a seed that has become more a passion than a career. Within therapy Gayle has several interests that are personal in nature. You'll find that Gayle has retained the compassion for her patients that she herself experienced while in recovery. Additionally, as the daughter of the elderly parents with health issues, Gayle carries a gentle touch and warmth to patient care for those with needs associated with aging. Her background in personal training has equipped her to be especially sensitive to your needs and limits as she develops exercise plans for both therapy sessions and home care.

Gayle is the mother of two grown children and the doting Nana to two - soon to be three - grandchildren who are the current focus of as much of her free time as possible.

 

Linda Emrich

Receptionist-Hamsphire.

Linda came to Moffett Physical Therapy in April of 2006. She has 17 years office experience. She graduated from Harper College in Palatine, IL with two Associates Degrees. Linda likes working with our Hampshire patients to make them feel as comfortable as possible. Outside of work Linda enjoys reading, boating and spending time with family and friends.

Cari McGuinn

Receptionist-Lake in the Hills.

Cari just moved to the area. She has been in the Customer Service field for 10 years. She is excited to be working at our new LITH clinic and meeting all the new patients. In her free time she loves to spend time with her family and 2 pugs.
