About Us

Moffett Physical Therapy provides high quality care in a comfortable friendly environment. Our slogan is “Building a Pathway to Better Health and Fitness.” Fitness is a stepping stone for good health and good health sometimes requires intervention from a skilled professional such as a physical therapist. Our physical therapists are experts in exercise prescription and joint and muscle dysfunction. We are physical therapist owned and proud to have three convenient locations just off Route 90, in Huntley, Hampshire and in Lake in the Hills.

What is a physical therapist?

Physical therapists are highly educated professionals, who are now graduating from Universities with a Doctorate of Physical Therapy. Physical therapists are experts in rehabilitation of muscles and joints, injury prevention and prescribing exercises that are appropriate to the individual. Our physical therapists are orthopedic trained. We treat all ages of people who have soft tissue and joint dysfunction.

What to expect...

Our physical therapists will do a thorough examination of your body and how it relates to your pain and symptoms. We will then determine a treatment plan unique to your dysfunction. This treatment program will help you obtain the best possible range of motion, strength and most importantly… function. During your entire treatment process we will communicate effectively with your physician in order to address your pain and symptoms with care and respect.

About the Owner, Cheryl A. Moffett, M.P.T.

  • Cheryl is a 1993 graduate of Northwestern University Medical School, School of Physical Therapy, Chicago, IL, with a Masters degree in Physical Therapy.
  • Cheryl completed her undergraduate at University of Washington, Seattle, WA, in 1991 with a Bachelor of Science degree in Zoology.
  • Cheryl also has had extensive training and coursework in:
    • Manual Therapy of spine and extremity
    • Craniosacral Therapy
    • Visceral Manipulation
    • Certification in pregnancy and exercise
    • Athletic training at a collegiate level
Cheryl has directed orthopedic physical therapy centers, provided acute care, and participated in specialized committees. She is a member of the American Physical Therapy Association and very active in the Huntley Area Chamber of Commerce. Cheryl is friendly and dedicated to her patients, always providing professional and quality care.

Dr. Deborah A. Mitchell, P.T., D.P.T., M.P.T., C.G.S.

Education:

  • Received her DPT from Temple University in Philadelphia, PA in 2006
  • Masters in Physical Therapy from Bradley University in Peoria, IL 2002
  • Bachelor of Science degree in Health Science from Bradley University in 2000.
  • Debbie has taken additional training in:
    • Vestibular Rehabilitation
    • Parkinson’s Rehabilitation
    • Balance and Fall Prevention
    • And is a Certified Golf Fitness Specialist

Debbie joined Moffett Physical Therapy in the early summer of 2006. Debbie says, "The most rewarding part of my professional career is the ability to help individuals return to his or her normal activities." Outside of work Debbie enjoys reading, gardening, walking her dog, golf, and spending time with friends and family.

Dr. Nicole Pump, P.T., D.P.T.

  Education:

  • Received a Doctorate in Physical Therapy from Northwestern University in Chicago, IL 2006.
  • Received a Bachelor of Science degree in Kinesiology with emphasis in Exercise Science and Fitness from Illinois State University in Normal, IL in 2004.

Nicole joined Moffett Physical Therapy in January of 2008. She enjoys working in the orthopedic setting. Her interests include treatment of TMJ, sports related injuries, and general rehab. Nicole plans to pursue a certification in manual therapy in the future as well as administer FCE's. She enjoys being a physical therapist because she is able to help patients reach their goals in order to live better lives. Nicole resides in Hampshire and enjoys spending time at the Sod Farm, watching, playing sports, and spending time with friends and family.

Dr. Kurt Van Alstine, P.T., D.P.T., A.T.C.

Education:

  • Received a Doctorate of Physical Therapy from Midwestern University in Downers Grove, IL in 2008.
  • Received a Bachelor of Science degree from Trinity International University in Deerfield, IL in 2000.

Kurt has been with Moffett Physical Therapy for over 3 years. Initially he was hired as a Physical Therapy Aide while finishing his graduate degree in physical therapy. He has been a Physical Therapist since November of 2008. Kurt also received his certification in athletic training in 2001 after graduating from Trinity International University. Kurt's interests in physical therapy stem from childhood experiences assisting with his mother's home therapy. As a Physical Therapist, Kurt enjoys listening, interacting and ultimately helping patients achieve their goals to improve their quality of life. Outside of work Kurt enjoys spending time with his wife and 2 kids. His interests also include exercising, camping and watching the Chicago Cubs and Bears games.

Elsie Jakubowski, P.T.

Education:
  • A 1997 Graduate of the Universidad Industrial de Santander in Colombia, South America with a Bachelor Degree in Physical Therapy

Elsie came to Moffett Physical Therapy in the January of 2006. Elsie speaks English and Spanish fluently. Outside of work Elsie enjoys watching movies, outdoor activities, spending time with her husband and two sons. Elsie says, "The most rewarding part of my professional career is being able to help people improve their quality of life."

Annette Parnell, P.T., M.S.

  Education:

  • Received a Masters of Physical Therapy from Northern Illinois University in DeKalb, IL in 2007.
  • Received a Bachelor of Health Services from Northern Illinois University in 2005.

In June of 2008, Annette joined Moffett Physical Therapy. Her interests in the orthopedic field have led her into the outpatient clinic. Annette has enjoyed specializing in women's health, pelvic floor, and bladder therapy due to the patient population as well as the unique challenges each case presents with. Annette believes in a "hands on approach" to manual therapy which is why she has started to work towards a certification in manual therapy at the Institute of Physical Arts. Annette loves the outdoors, especially running and biking on a nice sunny day.

Jenny Taylor, P.T., O.C.S.

  Education:

  • A 1980 graduate of Northwestern University, Chicago, IL with a Bachelor of Science degree in Physical Therapy.
  • Certified as a clinical specialist in Orthopaedic Physical Therapy by the American Board of Physical Therapy Specialties (ABPTS) in 2006.
  • Extensive experience treating spinal disorders using a manual therapy based approach.
  • She received Manual Therapy Certification (MTC) through the University of St. Augustine for Health Sciences in October of 2009.

Her other interset areas include treatment of temporomandibular, cervicogenic headache and shoulder disorders. She has had past experience in pain management, occupational health, industrial consultation and general rehabilitation. She is an active member of the American Physical Therapy Association (APTA) and the American Academy of Orthopaedic Manual Physical Therapists (AAOMPT). She is dedicated to continual improvement of her clinical skills in order to provide quality care for her patients. Jenny says, "I am thankful that I have always loved my work as a PT. Not everyone can say that about their chosen profession." Jenny and her family are the quintessential RV campers whose personal goal is to visit every National Park in the US.

Leena Varghese, P.T., M.P.T.

  Education:

  • Received a Bachelor of Arts degree in Psychology from Marquette University in Milwaukee, WI in 2002.
  • Received a Masters of Physical Therapy degree from Marquette University in 2004.

Leena has returned to Illinois after living and working in Boston over the last few years. She has taken additional training in functional exercise progressions, rotator cuff and ACL rehabilitation, elbow injuries in throwing athletes, and manual myofascial release techniques for different patient populations. Leena loves to spend time with her friends and family, especially her husband and son.

Carrie Lyne Mathiesen, P.T.A.

Education:

  • A 1998 High Honors Graduate of Elgin Community College in Elgin, IL with an Associate Degree of Applied Science in Physical Therapy.
  • Bachelor of Science in Biology, Bachelor of Arts in Applied Psychology at the Universty of Illinois at Chicago, May 1994.
  • Carrie Lyne also has taken additional training in:
    • Geriatric Orthopedics
    • Orthopedic Rehab of the knee and shoulder
    • Manual Myofascial Release Techniques
    • Certified Weight Trainer

In April of 2006 Carrie Lyne came to Moffett Physical Therapy with 8 years of outpatient orthopedic experience and works primarily at the Huntley clinic. "The best part of my profession is being able to not only help people feel better, but educate them on how to help themselves", says Carrie Lyne. She resides in Hampshire and enjoys sports, landscaping, decorating her home, cruising in her 1967 Ford Mustang and keeping active with her four children.

 

Danielle Kuhn, Physical Therapy Aide

 

Danielle came to Moffett Physical Therapy in January of 2008. She enjoys working with the patients and staff at Moffett. In her spare time she enjoys spending time with her niece Hailey. She also likes being outdoors and spending time with family and friends.

Laura Niemi, Physical Therapy Aide

 

Laura joined Moffett Physical Therapy in October of 2008. She enjoys working with patients and assisting the therapists. In her spare time she enjoys spending time with friends and family and likes to watch Bears games!

Loretta Quinlan, Accounts Receivable/External Marketing Manager

 

Loretta was very happy to join the Moffett Physical Therapy team in October of 2007. She brings over 10 years of Marketing and Accounts Receivable experience. She has lived in the Huntley area for over 20 years while raising her 2 children with her husband. Outside of work she loves working in her yard and taking floral design classes at McHenry County College.

Linda Emrich, Front Desk-Hampshire

 

Linda came to Moffett Physical Therapy in April of 2006. She has 17 years office experience. She graduated from Harper College in Palatine, IL with two Associates Degrees. Linda likes working with our Hampshire patients to make them feel as comfortable as possible. Outside of work Linda enjoys reading, boating and spending time with family and friends.

 

Shawn Hansen, Front Desk-Huntley

  Education:

  • Bachelor of Arts degree in Communications from Western Illinois University in 1994.

Shawn joined Moffett Physical Therapy in September of 2008. She brings over 11 years of experience in the employee benefits industry and also worked for several years in the real estate field. She enjoys meeting all of the patients and seeing the progress they make throughout therapy. Outside of work she enjoys spending time with her family, friends and her 2 dogs.

Kim O'Connor, Front Desk-Lake in the Hills

 

Kim joined Moffett Physical Therapy in November of 2008. She is married and with two children and a boxer. She loves the Cubs and enjoys spending time with her family and friends.

 

Cari McGuinn, Internal Marketing Manager

 

Cari joined Moffett Physical Therapy in March of 2007. She brings over 10 years of experience in the Customer Service field. She has enjoyed working at the Lake in the Hills clinic since its opening and is now focusing on our internal marketing. In her free time she loves to spend time with her family and welcomed a new baby girl this past November.
